Minkoff and Cline’s Comprehensive Continuous Integrated System of Care (CCISC) can be applied across teams, programs, organisations, and partnerships to enhance the delivery of integrated care. This approach incorporates processes and tools that address evolving challenges, allowing individuals at all levels – from frontline staff to leadership – to review practices within the current context and adapt or modify policies, procedures, and service delivery accordingly. CCISC encourages regular progress reviews and provides tools to measure and track changes.
The presentation will cover an overview of CCISC, its practical implementation with minimal funding, and the progress achieved over 12 months. It will include the presentation of change data and offer participants the chance to engage with the tools. The Access Health and Community and Banyule Community Health experience highlights how staff are empowered to identify and implement changes that improve integrated care.

Dr. Tamsin Short (She/Her) is the Executive Lead of Mental Health and Alcohol and Other Drug (AOD) at Access Health and Community, a leading community health service in Melbourne’s East. As a clinical and forensic psychologist, she has worked in a range of clinical and management roles over the past 15 years, including positions within AOD, forensic mental health and community health services.
