Achieving the vision of truly integrated care with a multi-disciplinary team

Service integration is a popular goal, but why is it so rare? Can we really break down the silos? First Step is a not-for-profit mental health, substance use and legal services centre. Against a siloed backdrop, our work crosses the sectors of health, mental health, AOD, legal, housing and community services. Our presentation will outline how First Step supports people with co-occurring needs including drug use, present data regarding the impact of our model on our clients, and explain how we achieve our results. While the benefit of people accessing GPs, addiction specialists, peers, nurses, mental health nurses, psychologists, group therapists and lawyers on one site may be obvious, this presentation will outline the challenges and some of the solutions to delivering truly integrated care.
