A bit about the role

The Community Health Manager will play a key leadership role in driving service delivery and program development across several critical services, including:

  • The Living Room (Drop-In & Primary Health Services)
  • Substance Misuse
  • After Hours Outreach Service
  • Collocated allied health services.

Based in the heart of Melbourne’s CBD tourist precinct, this is a unique opportunity to lead a team in delivering high-quality, integrated healthcare and support services to some of Melbourne’s most vulnerable adults.

You will be responsible for overseeing a multidisciplinary team that provides primary health services to individuals aged 18+ links them with vital services such as housing, AOD, mental health, and therapeutic support. As a Community Health Manager, you will lead evidence-based, operational responses and ensure that services are delivered effectively and efficiently, making a meaningful impact on individuals who are seeking pathways out of homelessness.

This is an exciting opportunity for an operational leader with a strong commitment to service excellence and a passion for making a difference in the community.

Key requirements:

  • Demonstrated experience (3+ years) leading a team with diverse experience.
  • Demonstrated experience (3+ year) in delivering clinical supervision to clinical and non-clinical teams.
  • Recognised Diploma in AOD, Community Services, Social Work or a tertiary qualification in relevant health related disciplines
  • Current First Aid level 2 certificate

A bit about us

The Living Room is a unique primary health service dedicated to improving the physical, mental, and social well-being of individuals aged 18+ who are experiencing or at risk of homelessness. As the only service of its kind in Melbourne, we focus on the social determinants of health, going beyond treating illness to consider the full context of a person's life. Our multidisciplinary team works with individuals to set goals that prioritise harm minimisation, self-determination, and empowerment.

As part of Youth Projects, an independent charity, we provide tailored support to people facing multiple challenges such as homelessness, unemployment, and substance misuse. We are committed to creating pathways out of poverty, operating 365 days a year to meet the complex needs of our clients.
