About the role

This position sits within the AOD Outreach cluster which aims to improve the health, wellbeing, housing and social outcomes of people who use alcohol and other drugs and/or those experiencing homelessness.

The Case Manager will carry a case load and will also contribute to the delivery of outreach harm reduction services to individuals and groups. This role will also support duty operations at cohealth's central city site.

Your duties include:

  • Undertake assertive street outreach within a multidisciplinary team
  • Carry a caseload of clients for case management
  • Contribute to whole of organisation initiatives to deliver cohealth's strategic directions and commitment to values-based care, co-design and community engagement

Our ideal candidate:

  • Minimum Certificate IV in a relevant discipline
  • Lived experience will be highly regarded
  • Sound knowledge of the health and social issues and service sector for people who use substances and experience homelessness

All appointments are made subject to a satisfactory Police Check, a Working With Children Check (WWCC), complete a Statutory Declaration, evidence of legal rights to work in Australia, and evidence of vaccination against COVID-19 (3 doses).

Successful applicants will be required to provide evidence of immunisation assessment including Influenza within 12 weeks of commencement [B]. This is in accordance with the Victorian Minister of Health Mandatory Vaccination Orders for COVID-19 and legislated requirement for influenza vaccination.
